The arrival of CAR T-cell therapies in multiple myeloma-idecabtagene vicleucel (ide-cel) and ciltacabtagene autoleucel (cilta-cel)-is an important advance but it brings with it the need to address patients who fail these treatments. The authors state that patients who are triple-class-refractory, meaning they are refractory after receiving a proteasome inhibitor, an immunomodulatory drug, and an anti-CD38 antibody, have a poor prognosis. Thus, the hunt for new therapies continues. As the investigators explained in the NEJM article, the explosion of treatment options in multiple myeloma means patients are living longer, but the downside is that nearly all will relapse. Talquetamab is being developed by Janssen, which paid for the study. So this is really life changing for these patients,” he said during a news briefing. “We don’t even know how long the remissions last. What Chari called “incredible” responses were maintained across important subgroups, including high-risk patients even patients with extramedullary disease saw response rates of 50%. “We’re hoping that this will soon become available so that patients can benefit.” This means that three-quarters of these patients are looking at a new lease on life,” said Chari. ![]() The mean time to best response was 2.7 months for the larger dose, given every 2 weeks. Both doses were given subcutaneously.Ĭhari emphasized during his presentation that patients responded quickly to talquetamab the median time to the first response was just over a month, with a median of 2.2 months to the best response for the lower dose tested given weekly. The combined data, which involved 288 patients with a median of 5 prior lines of therapy, produced overall response rates of 74.1% for patients treated a dose of 405 µg/kg weekly and 73.1% for patients treated with 800 µg/kg every 2 weeks, with a median duration of response of 9 months or longer. Results from phase 1 were simultaneously published in the New England Journal of Medicine ( NEJM). Ajai Chari, MD, professor of medicine and director of clinical research in the Multiple Myeloma Program, Mount Sinai, New York, New York, presented combined results from phase 1 and 2 from the MonumenTAL-1 study (NCT03399799) on December 10, 2022, at the 64th American Society of Hematology (ASH) Meeting and Exposition in New Orleans, Louisiana. More than 70% of patients with multiple myeloma had responses, and about a quarter had complete responses, to the bispecific antibody talquetamab even after progressing on a half-dozen other treatments-which for some included chimeric antigen receptor (CAR) T-cell therapy. Differences in deuterium uptakes were observed in the two regions where they were also detected in the adalimumab variants, and the conformational differences appeared to be an important factor for the low FcRn affinity of etanercept.More Than 70% of Heavily Pretreated Patients Taking Talquetamab for Multiple Myeloma See Responses That “Deepen Over Time” Although the Fc fusion proteins were expected to have similar FcRn affinity to IgGs, the affinity of etanercept to FcRn was lower than that of adalimumab, and its half-life was shorter than those of the IgG antibodies. Additionally, we investigated the conformational difference of Fc between a Fc fusion protein (etanercept) and a native IgG (adalimumab). The conformations around F245-L255 (FLFPPKPKDTL) were particularly influenced by the amino acid substitution in M256-P261, and the conformational changes of this region were correlated with the decrease of the affinity to FcγRIIIa. The amino acid substitutions in T254-P261 caused a change in deuterium uptake into some regions of Fc in HDX-MS analysis, but those at T311, M432 and N438 did not cause such a change. In this study, we investigated the binding property to FcγR and the conformation of seven FcRn affinity-modulated adalimumab variants to clarify the impact of the amino acid substitutions on the function and conformation of IgG Fc. To further prolong the half-lives, amino acid-substituted antibodies having high affinity to FcRn are being developed, and one such therapeutic antibody (ravulizumab) has been approved. Therapeutic immunoglobulin G (IgG) antibodies have comparatively long half-lives because the neonatal Fc receptor (FcRn) binds to the IgG Fc at acidic pH in the endosome and protects IgG from degradation. ![]()

Ham is a chimp who is the grandson of a famous chimp astronaut who was the first chimp in space. Opening the same weekend as The Dark Knight and Mamma Mia, we needed a good hearted film for the family as well, and Space Chimps does deliver on that. I agree that it's nothing special, but it's a cute movie that had it's moments. I was thinking how this movie was probably awful, but how could it be? It's an animation film, so I still wanted to see it, I checked it out today and honestly, it's not a 4.4 rating movie, I think people are being a little too harsh on it. I was first off a little surprised that it didn't do so hot at the box office, not only that, but it has a pretty low rating here on IMDb. If you must get it, stick to a rental for your young child. When I saw the trailer for Space Chimps, I honestly thought it looked cute and fun, also they mentioned that it was made by the same folks who did Shrek, so I figured that it was going to be gold. You don’t have to get this and it isn’t good enough to warrant passing up something from Pixar or Disney that you don’t have in your collection yet. The problem with Space Chimps is that there are just too many alternatives out there. It’s a shockingly small amount of extras and the lack of a commentary, making-of, or anything of that nature makes this an even worse value than normal. The trailers are listed individually on the menu so it seems like more, but they are very similar. That’s really all there is if you ignore the still gallery and the various trailers. It’s not particularly good, although I did get a kick on the producer and director saying that Cheryl Hines was helped by her improv background and then ten seconds later Hines saying doing the voice work was completely different than doing improv. A nine minute “Fox Movie Channel Presents Casting Session” featurette focuses on the voice actors and serves as the only behind-the-scenes info about the project. There is almost nothing else on the disc worth mentioning. I would never watch this movie, or any movie, in full screen since it is a sin for which there is no forgiveness, but at least you have the option and I’m all for choices. The best thing I can say about it is that it is one of those discs where you can flip it over and watch it either in widescreen (which you should) or full screen (which you should not). In a nice bit of symmetry, the lousy movie Space Chimps is given a pretty lousy DVD release. Use the money you would spend on this to buy a second copy of Wall-E for your other room. It will probably entertain a small non-discerning child while you fix dinner, but it’s not going to do anything at all for their parents or older siblings. Space Chimps is like one of those direct-to-DVD Disney movies. This leads to the usual clash that you see in movies like this, only in this case, the clash is between chimps who are going to go through a black hole. 5/1/2023 0 Comments Films that subvert genre![]() “All Rose is really saying in his article is ‘I don’t like horror, so these particular films must be something else,’” argued one online article, which concluded: “using platforms of cultural gate-keepership … to denigrate a persistently rich and popular genre with little mind to nuance is more than simple cultural distinction, it’s shoddy journalism.” Well that was me told. Responses to my article varied from “It’s all just horror, duh” to “This is elitist snobbery” to “You can’t possibly weigh on this subject because you haven’t watched as many horror movies as I have”. The “horror community” wasted no time in telling me how wrong I was. And in the same way postmodern architecture played with established language and traditions without necessarily sticking to the rules, I suggested a number of recent horror movies were doing the same: movies such as It Comes at Night, A Ghost Story and The Witch. To me, the term had a nice ring to it – a bit like “postmodern”. That said, Nope, did everything right from the moment the UFO appeared at the end when its true nature is revealed and deeper themes are made evident.F ive years ago, in this paper, I coined a term to describe what I saw as a new trend in horror movies: “post-horror”. It's an extremely human story that most wouldn't suspect, as most alien films focus on the spectacle of tragedy more than the people affected by it. The film is a think piece on exploitation told through the eyes of a family trying to stay afloat. As the film progresses, it's clear the object in question was alien in nature, but nothing could prepare the characters or audience for the truth behind the UFO in question.įrom the start, the film is played up as a classic UFO film, complete with a flying saucer, but as the story is expanded, it's more a story about humans versus a beast and showing how far humanity will go to cash in on something tragic. As they try to get back on their feet following their father's surprising death, it's revealed that something strange is hovering just above their ranch. The film followed OJ and his sister Em, who co-own a family business after their father died, where they train horses for movies. Peele's most recent film, Nope, has easily become his version of Jaws in all the best ways. Even though Us maintained its horror elements, it used the reel of doppelgängers to instead tell a deeply personal story for the main character while also exploring the world and how it has handled the idea of privilege. It also looked at equality in its purest form, where class and race didn't matter, but on the flip-side, it also showed humanity at its most primal as humans literally fight themselves to see which face comes out on top. ![]() In fact, the film was instead another bait and switched to explore ideas of privilege and control. ![]() While this foundation did remain, it wasn't the main focus. The film set itself up as an Invasion of the Body Snatchers-style story as families were killed by their Tethered and replaced. Confusion quickly becomes the story's main focus, as this family does their best to survive what is quickly revealed to be a rebellion of sorts. While there, things take a peculiar turn as doppelgängers invade their home and demand to take what's theirs. The film follows a family headed on vacation by a ocean for the summer. Us was Peele's sophomore film and likely his most out there yet. Instead, Peele used the concept of being isolated in an unknown environment and the anxiety of meeting a significant other's family and subverted the initial concept with a sci-fi twist that no one could've expected. However, this was only a small aspect of the larger theme and never played into the big surprise. This theme not only made sense from the perspective of the social climate and was even set up with particular scenes. ![]() RELATED: Nope Subverted Horror's Most Eye-Rolling Trope - and Made It Betterįrom the trailers, it appeared race would play a large role, as Chris was a Black man in an interracial relationship and was nervous about heading to a rural place to meet Rose's family.
